چکیده

significant reduction in morbidity and mortality rates in a wide range of critically ill patients when insulin therapy is used to maintain normoglycemia within a narrow range (80 to 110 mg/dl). Specific subpopulations in which glycemic control appears to be of particular importance include patients with cardiac disease, neurotrauma, or sepsis. The mechanism(s) associated with the detrimental effects of hyperglycemia and the improvement associated with strict glycemic control are currently under investigation. This article reviews the normal mechanisms of glucose control, causes of hyperglycemia in critically ill patients, potential detrimental effects of hyperglycemia, and implications for clinical veterinary practice.

The Geometry of Timed PV Programs

We introduce a real-time extension of the PV language: A timed PV program consists of a number of timed automata which synchronize by locking and releasing common resources. We give a geometric semantics to such programs in terms of local po-spaces, and we work towards making the established geometric techniques available for detecting deadlocks and unsafe configurations in timed PV programs.

Pathogenic IgG antibodies against desmoglein 3 in pemphigus vulgaris are regulated by HLA-DRB1*04:02-restricted T cells.

Pemphigus vulgaris (PV) is considered as a model for an autoantibody-mediated organ-specific autoimmune disorder. IgG autoantibodies directed against the desmosomal cadherin desmoglein 3 (Dsg3), the major autoantigen in PV, cause loss of epidermal keratinocyte adhesion, resulting in blisters and erosions of the skin and mucous membranes. The association of human autoimmune diseases with distinc...

A reciprocal HLA-Disease Association in Rheumatoid Arthritis and Pemphigus Vulgaris.

Human leukocyte antigens (HLA) have been extensively studied as being antigen presenting receptors, but many aspects of their function remain elusive, especially their association with various autoimmune diseases. Here we discuss an illustrative case of the reciprocal relationship between certain HLA-DRB1 alleles and two diseases, rheumatoid arthritis (RA) and pemphigus vulgaris (PV). RA is str...

Treatment with the Bcl-xL inhibitor ABT-737 in combination with interferon α specifically targets JAK2V617F-positive polycythemia vera hematopoietic progenitor cells.

Polycythemia vera (PV) treatment with interferon α (IFNα) is frequently limited by dose-related toxicity. PV CD34(+) cells are characterized by overexpression of Bcl-xL, which can be antagonized by ABT-737 leading to apoptosis. We explored the effects of ABT-737 and IFNα on PV hematopoiesis. Both IFNα and ABT-737 alone or in combination had a modest effect on normal hematopoiesis but each indiv...

Primary vs delayed surgery for spontaneous pneumothorax in children: which is better?

BACKGROUND Controversy exists regarding the timing of surgery for spontaneous pneumothorax (SP), which can be performed either after the first development of pneumothorax or after a recurrent spontaneous pneumothorax has occurred. Treatment after recurrence is often adopted because of the purported low recurrence of SP treated nonoperatively and the historical morbidity of open surgery. However...
